Online applications are invited for the following Grade- IV posts in the newly created Model Hospitals of the State under the establishment of theDirectorate of Health Services, Assam, as shown below, which will be available at the official website of Directorate of Health Services, Assam (www.dhs.assam.gov.in) from 14/11/2018 to 24/11/2018.
The number of posts may also increase in different Health Institutions which will be intimated in due course of time.

Age: Candidates should not be less than 18 years old and not more than 44 years on 03 March 2018. For SC / ST (P) / ST (H) candidates, the upper age limit may be comfortable for 5 years, OBC / MOBC and the person with a disability will be given 10 years (DIVYANG). Reservations of physically challenged persons (PWDs) will be according to the government. Do rule.

1. Interested candidates will have to fill the online application form and upload the documents, available on the official website of Health Services Directorate, Assam (www.dhs.assam.gov.in)
2. The following documents will be prepared at the time of document verification and photocopies of all the certificates etc.
(A) 02 (Two) Recently copies of passport size color photographs duly self-certified.
(B) Birth certificate for age proof.
(C) Physically challenged disability certificate (if any).
(D) Pass certificate.
(E) Caste Certificate (where applicable)
(F) Permanent Residential Certificate / Residential Certificate (if any).
(G) Employment Exchange Registration Certificate.
3. Detailed list of eligible candidates for each post along with the details of the examination will be notified as per the schedule and will be displayed on the website (www.dhs.assam.gov.in).
4. Tie-breaking of points: - In case of obtaining equal marks for two or more candidates, for such examples or case, age preference will be given i.e. old/senior candidates of age will be kept high in qualification.
